Dr. Patricia Panzica and Dr. John Hart are Board
Certified Pediatricians. They formed Pediatric Associates of Rockford in
1991.
Our Goal:
At Pediatric Associates of Rockford, the health and welfare of your
child is our top priority. Our goal is to provide your child with the
highest quality of care, while making the experience pleasant,
efficient, and friendly. We feel your child’s health is a partnership
between you and us. Our physicians are trained in the latest pediatric
advances. A physician is available 24 hours a day in the event your
child becomes ill.
Making Appointments:
Patients are seen by appointment only. Checkups are scheduled in the
morning and early afternoon. Appointments can be made for sick children
the same day you call our office. We try our best to stay on schedule,
but sometimes emergencies do arise. If we are seriously delayed, we will
notify you as soon as possible. Please assist us by being on time
(neither early nor late) for your appointment. We ask for your patience
if you have to wait. We know your time is valuable and hate to keep you
waiting.
Emergency Care:
If an emergency arises, please call 911. For urgent care, you
may call our office number 24 hours a day. If you call at a time other
that regular business hours, an answering service will contact the on
call doctor. Our doctors make every effort to return your call ASAP.
Please note that if you have not received a phone call from our doctors
within 30 minutes, please call 815-398-1527 and inform the answering
service.
Canceling Appointments:
We understand that sometimes it may be difficult to keep an
appointment. As a courtesy to others, we ask that you give our office a
24-hour notice. If you are running late for an appointment please call
as soon as possible. If you are running more than 20 minutes late, the
exam may need to be rescheduled.
Referrals: It is
your responsibility to notify us if you need a referral. Since insurance
companies are no longer accepting backdated referrals, please request
your referral as soon as you schedule an appointment with a specialist.
Most referrals cover a 3 or 6 month time period. Please keep track of
when you will need to have your referral renewed.
Fees: Charges for
our office vary with the severity and complexity of the problem, and
time spent with your child. Payment is required at time of service. We
accept cash, check, or credit card. As a courtesy, we bill primary
insurance for you.
Doctor / Patient Relationships:
A healthy Doctor / Patient relationship is best developed when based
on mutual understanding. It is helpful to make a list of questions prior
to your appointment so that we can address all of your concerns. We make
special efforts to explain everything to you regarding your child’s
medical condition, medicines, and treatment.
If something is unclear to you, don’t hesitate to ask us about it. If
you have any suggestion regarding services or fees, please convey them
to us so we can better serve you. If you are happy with our services, please
tell your friends, as patient’s parents are some of our best
advertisers.
